The project aims to create a comprehensive medication reconciliation process for the hospital to improve patient safety, reduce medication errors, and ensure correct medication administration.

Community background

Patients receiving healthcare services in the hospital setting are the affected community and are at risk of experiencing medication errors.

The project aims to improve patient safety and reduce the likelihood of medication errors through a comprehensive medication reconciliation process, with hospital staff also involved in ensuring its success.

Literature Review

Problem/topic

Medication reconciliation is crucial for safe and effective healthcare and is associated with improved medication safety, decreased medication errors, and improved patient satisfaction.

Education and training in medication reconciliation are necessary for successful implementation in healthcare settings.

Implementing a comprehensive medication reconciliation process can reduce medication errors, improve patient safety, and lower costs related to adverse drug events.

Solution/Intervention

Daliri et al. (2021) found that longitudinal medication reconciliation decreases medication mistakes and adverse drug events.

Killin et al. (2021) conducted a systematic review and found that advanced medication reconciliation helps reduce medication errors and adverse drug events.

Both articles demonstrate the effectiveness of medication reconciliation in improving patient safety and the need for education and training in medication reconciliation to ensure successful implementation in healthcare settings.

Develop and implement a comprehensive medication reconciliation process to improve patient safety and reduce medication errors in the hospital setting.

Identify a healthcare facility setting with systemic issues in medical reconciliation systems and target it for improvements.

Evaluate the current medication reconciliation process and identify areas for improvement.

Develop a comprehensive medication reconciliation process.

Follow up and evaluate the impact of the medication reconciliation process.

Assessment

Develop an evaluation plan to assess the impact of the medication reconciliation process.

Collect data on medication errors, adverse drug events, and patient satisfaction to assess the impact of the process.

Survey staff to evaluate the effectiveness of the process and identify any areas that need improvement.
